Nominate Your Favorite Advocate


A nomination form is located below - it includes instructions and a brief write up on the criteria for each of the awards.

Awards will be presented on Monday, April 28, 2008 at the annual conference's opening luncheon to be held at Springfield's Prairie Capitol Convention Center.

If you have questions about the awards nomination process, please call the CCDI office at (800) 433-8848 (V/TTY). Don't forget, all nominations must be in to the CCDI office, no later than March 24, 2008!

The 24th Annual Disability Rights/Independent Living Conference will be held on April 28-30, 2008 at Springfield's Prairie Capitol Convention Center. The upcoming event provides the disability community an opportunity to honor those among us who have displayed perseverance and leadership in the struggle for equal rights.

CCDI traditionally honors several advocates each year and we invite you to nominate anyone you feel fits or exceeds the criteria listed below. Please use the form provided to nominate a CCDI member for any of these awards. Submit your nominations no later than March 24, 2008.

I nominate the individual identified below for the following award:

___The Doctor Judy Smithson Advocacy Award recognizes an individual for advocacy activities supporting the human and constitutional rights of citizens with disabilities. The award recipient should be someone who has advocated on more than one issue of concern. The individual should be a positive role model for others to follow.

___ The Mary Linden Award honors a member of the Coalition who has demonstrated conviction, dedication and strong personal growth. This is a person who volunteers their time, energy and talents in pursuit of the mission and goals of the Coalition.

___ The Markeeta Award is presented to an individual for their advancement and progress as a youth in pursuit of social justice.

Please attach a statement about your nominee and describe why you believe he or she would be a good candidate to receive the award you've indicated above. Be as descriptive as you like. If you have any questions about the nomination process, call the CCDI office at (800) 433-8848 (V/TTY).
